Family history, medical history, allergies, current medications, physical condition and complaints, etc.
Past psychiatric history, trauma, psychiatric hospitalizations, past psychotropic medications, history of mania or hypomania, psychosis, depression, anxiety, panic, phobia, obsessions, inattention, impulsivity, self-harm, violence, eating disorder, gambling, sexual disorder, goals and hopes, primary care doctor, therapist, substance use, etc.
